    Studies have suggested that DSE is superior to exercise ECG for diagnosis of CAD in hypertensive patients16 and has both diagnostic17 and prognostic value4 in renal patients. In head-to-head studies comparing ischemia by DSE and angiographic CAD, the reported sensitivities for prediction of coronary stenosis by DSE range from 75% to 95%,5,17,18 which is better than in studies comparing SPECT with angiography.19 In the two largest prognostic series, the “warranty” of a normal DSE in CKD appears to be limited to about 2 years after which time the event rate more than doubles.4,20

    In patients in AF there is a high risk of thromboembolism as determined by the CHA2DS2VASc,18 this is the case in most studies that have analyzed this issue. In our study the risk of thromboembolism may even be underestimated, since we have not included as a variable the presence of heart failure, since there was no echocardiogram available to document ventricular function and we consider that the clinical diagnosis of heart failure in the dialysis population is difficult in a study of these characteristics.19 In the studies conducted in our institution, patients with AF present a high rate of thromboembolic events and mortality as compared to patients in sinus rhythm.13,20,21
